Job Description:

Assists in the continuity of care by monitoring patient data. Facilitates communication with the patients to the Director of Nurses and Provider offices. Responsibilities include:

  • Building relationships with patients for: service acceptance, device kitting and fulfillment.
  • Building relationships with our partner physicians, ACOs, and Hospitals to establish and adhere to continuum of care processes
  • Assist in the creation and fulfillment of research and clinical grants for telemonitoring and in the creation/fulfillment of DSRIP initiatives with partner hospitals and communities
  • Provide behavioral coaching and enroll in adhoc behavioral training sponsored or provided by Accuhealth
  • Make 100+ calls per shift to patients with critical readings.
  • Monitor patient vitals and report concerns to the supervisor as necessary.
  • Provide supervision to Medical assistants
  • Train clients on our platform and processes.
  • Troubleshoot devices
  • Other duties as assigned.

Job Requirements:

  • Must be nationally registered or certified as a Medical Assistant
  • Strong phone and verbal communication skills along with active listening
  • Familiarity with Google Suite, ability to quickly learn and properly use several systems for documentation and communication
  • Customer focus and adaptability to different personality types
  • Ability to multitask, set priorities and manage time effectively
  • Fluent in Spanish and English, highly preferred
  • Previous call center experience, highly preferred

Technology Requirements for the role:

  • Internet upload and download speeds need to be a minimum of 35 mbps

Pay: $15.00 - $16.50 per hour
